I. Teaching Objectives
Learn to design a lifestyle survey including aspects like diet, rest, exercise, and happiness - related activities, using a model survey as reference.
Develop collaborative skills by working in groups to brainstorm questions and discuss possible answers.

III. Teaching Procedures

Step 1 Answer the model survey (Step 1)
Individual work: Model survey
T: Answer the questions in the model survey “How healthy is your lifestyle?”.
Ask Ss to answer the model survey questions individually to familiarize themselves with the survey format and content.
Step 2 Brainstorm survey questions (Step 2)
Group work: Question brainstorming
T: Work in groups. Brainstorm a list of questions for your lifestyle survey, including diet, rest, exercise, and things that make you happy.
Divide Ss into groups. Ask each group to brainstorm survey questions, guiding them to ensure coverage of the required aspects. Invite some groups to share their questions.
Step 3 Discuss possible answers (Step 3)
Group work: Answer discussion
T: Share your ideas. Discuss possible answers to the questions in your survey. Use “TEEN SKILL” expressions to invite others to share views.
Ask each group to discuss possible answers for their questions, using expressions like “Let’s share our ideas together.” “What’s your opinion of...?”. Walk around to provide help and encourage interaction.
Step 4 Conduct the survey (Step 4)
Class activity: Survey conduction
T: Do the survey in class. Each group conducts their survey with classmates, using the prepared questions and answer options.
Guide groups to conduct their surveys, ensuring smooth interaction and accurate question - asking.
Step 5 Record answers (Step 5)
Group work: Answer recording
T: Record the answers from the survey. Organize the data in a table (like “Results of survey” in the model).
Ask groups to record the survey answers systematically, preparing for result analysis and reporting.
Step 6 Report survey findings (Step 6)
Group presentation: Findings report
T: Report what you find in your survey. Try to provide some explanation for the findings and give some advice.
Invite each group to present their survey findings, explaining the results and giving advice (e.g., “Among them, __ people got mostly A’s. They should eat the right food and exercise regularly.”). Encourage classmates to ask questions or share opinions.
IV. Assessment of Teaching Effectiveness
Assess groups' collaboration and question - designing skills by observing their work in Steps 2 and 3.
Observe whether groups can conduct the survey smoothly and record answers accurately in Steps 4 and 5.
Evaluate groups' presentation skills in Step 6, checking if they can report findings clearly, explain results, and give appropriate advice.
This project helps Ss learn to design and conduct a lifestyle survey, improving collaborative, communicative, and analytical skills, and enhancing their understanding of healthy lifestyles.
V. Design Purpose
Answering the model survey familiarizes Ss with the survey structure and content, preparing them for designing their own.
Brainstorming and discussing questions/answers train Ss' collaborative and creative thinking, helping them plan the survey content and practice interaction.
Conducting, recording, and reporting the survey provide a practical experience, allowing Ss to apply knowledge about healthy lifestyles, practice data collection and analysis, and enhance communication through presenting findings.

VIII. Teaching Reflection
The project effectively engaged Ss in collaborative survey design and conduction. Groups showed good creativity in preparing questions and discussing answers. However, some Ss might have faced difficulties in analyzing data and giving clear advice during reporting. In future projects, more guidance on data analysis and presentation can be provided.
