On a school trip to New York, our school had us wear name tags (牌) with our school name on it. Since all the attractions we visited were popular with school groups, the tags let the teachers know we should be with our group. If any of us separated from the group, teachers would quickly call us back.
Some students had tried to hide or throw away their name tags to avoid this, so our head teacher—a very strict lady—said that anyone found without the name tag would have to spend the rest of the trip staying with her at all times and then have a month’s worth of lockdown (禁闭) when we got back.
While we were all on a ferry (渡船), we were allowed to go around. While playing with friends, I didn’t realize it was my name tag until it was too far to reach. I started to freak out at once. I had never got into trouble at school before or had even a single punishment, and I would possibly face a month’s worth of them! After thinking for a moment, I finally decided to just tell the head teacher what had happened.
I went to the head teacher and explained very nervously to her. Instead of shouting at me, she took out another name tag and handed it to me. She said, “I have prepared several more name tags in case something unexpected happens.” Then she smiled at me, “Did you think you would get lockdown? Don’t worry, and that’s just for the trouble-makers in order to stop them losing it on purpose. I believe you’re a good kid. Don’t be worried about this. But be careful later.” When I heard these words, tears came to my eyes. What a kind teacher she is!

Casting blame is natural: it is tempting to fault someone else for a mistake rather than taking responsibility yourself. But blame is also harmful. It makes it less likely that people will admit mistakes, and thus less likely that organizations can learn from them. Research published in 2015 suggests that firms whose managers pointed to external factors to explain their failings underperformed companies that blamed themselves.
Blame culture asks, “who dropped the ball?” instead of “where did our systems and processes fail?” The focus is on the individuals, not the processes. It’s much easier to point fingers at a person or department instead of doing the harder, but the more beneficial, exercise of fixing the root cause, so the problem does not happen again.
The No Blame Culture was introduced to make sure errors and deficiencies (缺陷) were highlighted by employees as early as possible. It originated in organizations like hospitals, submarines and airlines. Because errors can be so disastrous in these organizations, it’s dangerous to operate in an environment where employees don’t feel able to report errors that have been made or raise concerns that deficiencies may turn into future errors. The No Blame Culture maximizes responsibility because all contributions to the event occurring are identified and reviewed for possible change and improvement.
The National Transportation Safety Board (NTSB), which supervises air traffic across the United States, makes it clear that its role is not to assign blame or legal responsibility but to find out what went wrong and to issue recommendations to avoid a repeat. The proud record of the airline industry in reducing accidents partly reflects no-blame processes for investigating crashes and close calls. The motive to learn from errors also exists when the risks are lower. That is why software engineers and developers routinely investigate what went wrong if a website crashes or a server goes down.
There is an obvious worry about embracing blamelessness. What if the website keeps crashing and the same person is at fault? Sometimes, after all, blame is deserved. The “just culture” developed by James Reason, addresses the concern that the incompetent and the malevolent (恶意的) will be let off the hook. The line that Britain’s aviation regulator draws between honest errors and the other sort is a good starting-point. It promises a culture in which people “are not punished for actions or decisions taken by them that go with their experience and training”. That narrows room for blame but does not remove it entirely.

Fishy Weather Conditions
Laj amanu, Australia, is a dry little town, sitting right on the edge of the Tanami desert. Can you imagine how surprised people were when live fish rained down on them from a dark gray cloud. It happens that there are similar cases in England and Honduras.
How do clouds make fishy? The simple answer is that they don’t. There is a particular weather phenomenon (现象) called a waterspout. A waterspout is just like a tornado (龙卷风), only it forms above oceans, lakes, or rivers. Like a tornado, a waterspout moves in a circle at high speeds. When it moves above the water, it tends to carry the fish with it, as well as frogs or other small plants or animals.
Scientists couldn’t work it out at first. To make matters stranger still, the fish in Honduras were very much alive when they rained down to the ground, but they were all blind. In England and Australia, it rained fish and snakes, and none were blind. It was difficult to puzzle out, but the blind fish gave them a place to start.
Scientists knew that some fish that lived in deep, underground caves with no light sources often lost their eyesight. So when blind fish rained down on Honduras, scientists began to connect some clues (线索,提示). Clearly, these particular fish were pulled from an underground water source by force.
It has rained fish on every continent, and each time, people have tried in various ways to explain this strange phenomenon. Historically, villagers thought the “fishes from the heavens” might be answers to prayers (祈祷) for food. Others suggested that floods overran river banks and oceans, leaving the fish on the city streets. No scientist had actually seen the rain as it occurred, only the fish left on the ground. But in 1990, a National Geographic team happened to be in Honduras when the Rain of Fishes began. They recorded what was happening and made history by finally proving that the fish really did fall from the sky.
This huge breakthrough wasn’t just a spot of good luck. It changed thousands of years of myths (虚构的事;谬论) and legends into true stories and provided scientific explanations for how fish came to live in deep caves. It explained ancient cave paintings and cast (投射光线) new light on how species have spread over time. It turned out to be a lot more than just a little fishy weather.

It makes it less likely that people will admit mistakes, and thus less likely that organizations can learn from them.
