Trust me — I am expedition doctor
Ever since I was a young child, I’ve had a taste for adventure, but I never imagined I’d be able to satisfy this passion at regular intervals because of my chosen career.
My work as an expedition (探险队) doctor has taken me all over the world. However, my favourite trips, and the ones in which I now specialise, are those involving mountains. Never do I feel more inspired by nature than when I look up at their towering peaks and begin to prepare myself mentally for the challenges ahead.
I trained as a doctor in the UK, but there was little in that training to prepare me for binding up a broken leg during a storm on the side of a mountain! In fact, I’d say that medical skills come some way down the list of job requirements, after endurance, flexibility, problem-solving and communication.
This kind of medicine is a million miles away from the controlled, germ-free environment of a hospital, and your medical kit basically consists of whatever you can carry, so you sometimes have to be prepared to improvise (即兴做). For example, I’ve learned that some drugs can be used for several conditions, and I’ve even had to resort to cutting branches off a small tree to make a stick to support a broken arm.
That isn’t to say that you can’t train to be an expedition doctor; on the contrary, there are some excellent courses available. Not only do they teach medical techniques, but also practical skills such as carrying out risk assessments, crossing rivers safely and using satellite phones. The first course I did included a session on expedition dentistry, though I must admit I still don’t like the idea of pulling out someone’s tooth!
I do most of my work for adventure holiday companies, travelling to remote places. When I started out, these holidays were quite rare, but they have become much more mainstream now that we’ve all seen celebrities climbing Kilimanjaro or watched reality shows about people surviving in jungles.
I do have mixed feelings about all these people with large amounts of disposable income coming to poor areas just for their own enjoyment, so I try to make sure that the companies I work for have high ethical standards and benefit the local communities. And of course, tourism provides employment, and also opens the eyes of rich visitors to the hardship that many people are forced to endure.
I realise that this kind of life isn’t for everyone, but I’d recommend that all doctors try it at least once, if only to make them appreciate the comforts of their usual working environment!

Happiness, as I see it, comprises five elements: spiritual well-being (meaning and purpose), physical well-being (nutrition, exercise), intellectual well-being (curiosity, deep learning), relational well-being (kindness and generosity), and emotional well-being (cultivating positive emotions). As an interdependent aggregate of these five elements of SPIRE, happiness is about much more than experiencing pleasure.
As Aristotle put it, happiness is the ultimate purpose of life, meaning how we spend our everyday lives is ultimately guided by what we think would make us happier. This is not a good or a bad thing. It simply is, like the law of nature. Even people who are tirelessly working for an important cause, for example, to get rid of world hunger, are doing it because they find their work meaningful. Meaning is an element of happiness.
One barrier to happiness has to do with the expectation that happiness is an unbroken chain of positive emotions. This expectation, however, prevents people from experiencing happiness because painful emotions don’t go away but grow stronger when we reject them.
The second barrier has to do with equating happiness with success. It’s a commonly held belief that happiness can be attained by achieving certain goals, like money or fame. People tend to think if they finally find success, they will automatically become happy.
The third barrier has to do with the way people pursue happiness. We want to be happy for many reasons. After all, we are constantly told that happiness is good for our health, relationships, and work outcomes. Yet, if I wake up in the morning and decide to pursue happiness straight, I will become less happy.
But how? Indirectly. As is known, if you look up at the sun directly, you’ll hurt yourself. But if you take the same sun rays and break them down, you’ll enjoy the colors of a rainbow. Similarly, pursuing happiness directly can hurt us; pursuing it indirectly—by breaking it down into something like the SPIRE elements—can contribute to our well-being. Starting a meditation practice, exercising, performing acts of kindness, learning something new, or expressing gratitude for what we have are all indirect ways of pursuing happiness.

When a chunk of ice fell from a collapsing glacier(冰川)on the Swiss Alps’ Mount Eiger in 2017, part of the long deep sound it produced was too low for human ears to detect. But these vibrations held a key to calculating the ice avalanche’s(崩塌)critical characteristics.
Low-frequency sound waves called infrasound that travel great distances through the atmosphere are already used to monitor active volcanoes from afar. Now some researchers in this field have switched focus from fire to ice: dangerous blocks snapping off glaciers. Previous work has analyzed infrasound from snow avalanches but never ice, says Boise State University geophysicist Jeffrey Johnson. “This was different,” Johnson says. “A signature of a new material has been detected with infrasound.”
Usually glaciers move far too slowly to generate an infrasound signal, which researchers pick up using detectors that track slight changes in air pressure. But a collapse—a sudden, rapid breaking of ice from the glacier’s main body—is a prolific infrasound producer. Glacial collapses drive ice avalanches, which pose an increasing threat to people in mountainous regions as rising temperatures weaken large fields of ice. A glacier “can become detached from the ground due to melting, causing bigger break— offs,” says University of Florence geologist Emanuele Marchetti, lead author of the new study. As the threat grows, scientists seek new ways to monitor and detect such collapses.
Researchers often use radar to track ice avalanches, which is precise but expensive and can monitor only one specific location and neighboring avalanche paths. Infrasound, Marchetti says, is cheaper and can detect break—off events around a much broader area as well as multiple avalanches across a mountain. It is challenging, however, to separate a signal into its components (such as traffic noises, individual avalanches and nearby earthquakes) without additional measurements, says ETH Zurich glaciologist Malgorzata Chmiel. “The model used by Marchetti is a first approximation for this,” she says. Isolating the relevant signal helps the researchers monitor an ice avalanche’s speed, path and volume from afar using infrasound.
Marchetti and his colleagues are now working to improve their detectors to pick up more signals across at-risk regions in Europe, and they have set up collaborations around the continent to better understand signals that collapsing glaciers produce. They are also refining their mathematical analysis to figure out each ice cascade’s physical details.

Ever since I was a young child, I’ve had a taste for adventure, but I never imagined I’d be able to satisfy this passion at regular intervals because of my chosen career.
My work as an expedition (探险队) doctor has taken me all over the world. However, my favourite trips, and the ones in which I now specialise, are those involving mountains. Never do I feel more inspired by nature than when I look up at their towering peaks and begin to prepare myself mentally for the challenges ahead.
I trained as a doctor in the UK, but there was little in that training to prepare me for binding up a broken leg during a storm on the side of a mountain! In fact, I’d say that medical skills come some way down the list of job requirements, after endurance, flexibility, problem-solving and communication.
This kind of medicine is a million miles away from the controlled, germ-free environment of a hospital, and your medical kit basically consists of whatever you can carry, so you sometimes have to be prepared to improvise (即兴做). For example, I’ve learned that some drugs can be used for several conditions, and I’ve even had to resort to cutting branches off a small tree to make a stick to support a broken arm.
That isn’t to say that you can’t train to be an expedition doctor; on the contrary, there are some excellent courses available. Not only do they teach medical techniques, but also practical skills such as carrying out risk assessments, crossing rivers safely and using satellite phones. The first course I did included a session on expedition dentistry, though I must admit I still don’t like the idea of pulling out someone’s tooth!
I do most of my work for adventure holiday companies, travelling to remote places. When I started out, these holidays were quite rare, but they have become much more mainstream now that we’ve all seen celebrities climbing Kilimanjaro or watched reality shows about people surviving in jungles.
I do have mixed feelings about all these people with large amounts of disposable income coming to poor areas just for their own enjoyment, so I try to make sure that the companies I work for have high ethical standards and benefit the local communities. And of course, tourism provides employment, and also opens the eyes of rich visitors to the hardship that many people are forced to endure.
I realise that this kind of life isn’t for everyone, but I’d recommend that all doctors try it at least once, if only to make them appreciate the comforts of their usual working environment!

Happiness, as I see it, comprises five elements: spiritual well-being (meaning and purpose), physical well-being (nutrition, exercise), intellectual well-being (curiosity, deep learning), relational well-being (kindness and generosity), and emotional well-being (cultivating positive emotions). The second barrier has to do with equating happiness with success. It’s a commonly held belief that happiness can be attained by achieving certain goals, like money or fame. People tend to think if they finally find success, they will automatically become happy.
The third barrier has to do with the way people pursue happiness. We want to be happy for many reasons. After all, we are constantly told that happiness is good for our health, relationships, and work outcomes. Yet, if I wake up in the morning and decide to pursue happiness straight, I will become less happy.
But how? Indirectly. As is known, if you look up at the sun directly, you’ll hurt yourself. But if you take the same sun rays and break them down, you’ll enjoy the colors of a rainbow. Similarly, pursuing happiness directly can hurt us; pursuing it indirectly—by breaking it down into something like the SPIRE elements—can contribute to our well-being. Starting a meditation practice, exercising, performing acts of kindness, learning something new, or expressing gratitude for what we have are all indirect ways of pursuing happiness.

It is challenging, however, to separate a signal into its components (such as traffic noises, individual avalanches and nearby earthquakes) without additional measurements, says ETH Zurich glaciologist Malgorzata Chmiel. “The model used by Marchetti is a first approximation for this,” she says. Isolating the relevant signal helps the researchers monitor an ice avalanche’s speed, path and volume from afar using infrasound.
Marchetti and his colleagues are now working to improve their detectors to pick up more signals across at-risk regions in Europe, and they have set up collaborations around the continent to better understand signals that collapsing glaciers produce. They are also refining their mathematical analysis to figure out each ice cascade’s physical details.
