Dear Bob,

I am terribly sorry for not being able to keep my appointment to accompany you to the bookstore as scheduled. I'm writing to convey my heartfelt / genuine apology for breaking my promise.

It's because I have an important meeting to attend that I have no alternative but to miss the valuable chance. As you know, I have been looking forward to going to the bookstore with you since we made the appointment. Unexpectedly, the classmate who should have attended the meeting suddenly fell ill.
